There is a non-refundable application fee of $100 that must be paid at the end of this application. 

A letter confirming receipt of your application and further instructions will be mailed to the address listed in your application.

Education Completed
Have you ever been arrested and/or convicted of a felony, including the receiving of a Suspended Imposition of Sentence, following a plea of guilty to a felony charge?
Have you ever been arrested and/or convicted of a misdemeanor to include a DWI, BAC, Leaving the Scene?
Have you ever been involved in, arrested and/or convicted of a misdemeanor or felony in this or any other state?

After clicking submit, you will be automatically transferred to a new page to pay your testing fee of $100.

TESTING WILL NOT BE SCHEDULED IF THIS FEE IS NOT PAID.
