There is a non-refundable application fee of $100 that must be paid at the end of this application.

 

Please ensure all information is correct. The information you provide on this form will be used for your academy nametag, certificate of completion, and summary sheet to be sent to the Department of Public Safety. The contact information provided will be used to contact you throughout the application process. A letter confirming receipt of your application and further instructions will be mailed to the address listed in your application. You may also be contacted by email.

Education Completed
Have you ever been arrested and/or convicted of a felony, including the receiving of a Suspended Imposition of Sentence, following a plea of guilty to a felony charge?
Have you ever been arrested and/or convicted of a misdemeanor to include a DWI, BAC, Leaving the Scene?
Have you ever been involved in, arrested and/or convicted of a misdemeanor or felony in this or any other state?

SOCIAL MEDIA BACKGROUND CHECK AUTHORIZATION
 

I understand Mineral Area College Law Enforcement Academy will be conducting a Social Media Background Check, which will include a review of my public posts on social media as well as publicly available articles from the web and news sources.
 

Internal disciplinary action, including dismissal from the academy selection process, may be undertaken if you refuse to answer or if you fail to reply fully and truthfully.  Neither your answers nor any information or evidence gained by reason of your answers will be released without expressed authorization.

By typing my name below, I authorize Mineral Area College Law Enforcement Academy and its designated agents and representatives to conduct a comprehensive review of my social media accounts to determine my eligibility for the Law Enforcement Academy.

I hereby certify that the above information is true, correct, and complete based on my personal knowledge and belief.  I understand that providing false or fraudulent information or the omission of the requested information may be grounds for adverse action, up to and including disqualification from admittance or further attendance in this Academy, and may adversely impact future employment opportunities.
